at the level of the xxx muscles... Explanation: I'm no expert, I'm afraid, but have a suggestion. Perhaps it means the muscles are used as a reference for the level ("se toman como referencia"). There's an example in a book on a vitrectomy, which is related to the procedure for a detached retina. See Kanski's Clinical Ophthalmology E-Book: A Systematic Approach, page 717, on Google Books. TOMA DEL MUSCULO Una vez se tiene una zona de la esclera descubierta se introduce por ella el gancho. No hay que introducirlo a gran profundidad ya que se corre el riesgo de capturar grasa orbitaria. Si se calculan los milímetros a los que la inserción muscular está situada con respecto al limbo simplemente se entra un poco por detrás. El gancho se introduce con la punta siempre rozando la esclera para así evitar que la punta del gancho levantada lo haga salir a través del músculo. El gancho pasa por debajo del músculo, entre éste y la esclera. Si no se desliza suavemente nos podemos ayudar con una pinza que tracciona el tejido por encima de donde pasa el gancho para así dejarle más holgura. Una vez ha pasado todo el gancho por debajo del músculo se eleva la punta del mismo y en este momento es fundamental comprobar que todo el músculo está cogido y no ha quedado ninguna fibra suelta. https://books.google.co.uk/books?isbn=1455712337 George L.
