: This is simply about the addition of a name and nothing indicates that it is double-barrelled other than your imagination and an unknown degree of probability that isn't a translator's concern. What if it's a triple-barrelled name? WE SIMPLY DO NOT KNOW.

It would be unusual for a 'name addition' not to be a double- barreled sequel. 'M.X prend le nom de M.Y ' query: Mr. X-Y.

So the additional name will be Duval or Dupont, a surname, not Jean or Marie, a first name.
This is a distinction we would also make in English -- I don't think I'm being pedantic (for once?).
As for "declaration" versus "statement", my gut feeling is that kings and queens and governments make declarations, but we ordinary folk make statements.

For those of you who do enjoy a bit of pedantry, this is the name of a form, so it should be:
"Joint Statement of Surname Addition".
The Statement is then probably attached to the Birth Certificate and filed away.
So the additional name will be Duval or Dupont, a surname, not Jean or Marie, a first name.
