It's a direction given in a script => "A metallic CLANG (OS) occurs."

Although I have been working in the field for years, it's the very first time I've seen this (OS). Have no clue.

También se usa mucho esta expresión en lenguaje televisivo o fílmico. (Voz en off, sonido en off, etc.).

¡Fijate Daniela si te sirve, y aprovecho para saludarte! (mientras me tomo un mate en off).

Si es una pelicula Off Screen (como cuando una actor habla pero no se le enfoca)

agree James Stevens-Arce : OS is a term taken from the theater which has long been used in feature films to indicate that the action takes place off-camera. Sometimes OC (off-camera) is used instead.
