English to Spanish Medical Medical: Dentistry Sedation

DISCLORURE &CONSENT FOR ORAL OR IV SEDATION

The procedures to be performed have been explained to me and I understand what is to be done. This is my consent to the procedures discussed and to any other procedures found to be necessary or advisable in addition to the pre-operative treatment plan. I agree to the use of local, oral sedation, or IV sedation depending on the judgment of XXXXX Sperviising Doctors. I understand that treatment may be DELIVERED by a student dentist under the direct supervision of licensed dentist.

The translations I've used for DELIVER are either Ofrece, Facilita, Poporciona, but none of these seem to convey the meaning of the student DELIVERING the treatment. Woud it be okay to say: Yo comprendo que el tratamiento puede ser REALIZADO por un estudiante de odontología bajo la supervisión directa de un odontólogo autorizado.....
This is the first time I deal with Dentistry translations and am a little confused.
