The ward team had first to liaise with the crisis team who were first to respond and assess Carl’s situation and the early intervention team became part of Carl’s treatment team long before he was discharged. All the team members across the services worked closely together, in collaboration with Carl, in exploring future options and planning to meet his needs in order that he might be able to move on from the hospital as quickly as possible. In this instance the seamless service aspired to in the guidance documents was evident and Carl’s experience followed an optimal pathway (Birchwood et al 2001).
