with presence of disc tissue/material/extrusion
My understanding is that material from the disc is visible, suggesting a herniation or rupture. I think the text is assuming 'tissue', 'material', or a similar word, as we would expect in English. However, the author is only observing, not diagnosing a hernia as such.

and disc
I believe the author used some abbreviation there. There seems to be two medical conditions - osteophytosis and disc xxxxx. The problem is to identify what is xxxxx.
It could be (spinal) disc herniation, disc space narrowing, disc degeneration...
I believe you should contact your client to be sure about that (unless someone comes up with a better explanation for this occurrence of "disco").

Discussion
"Redução do espaço intersomático" is a typical sign of a ruptured/herniated disc - the disc tissue is being squeezed out of its normal space because the space has narrowed. At the same time, the author emphasizes that there is no "repercussão sobre a medula," meaning that the disc tissue has not affected the medulla (could be the spinal cord or brain stem, depending on the level). So the author is suggesting a herniated disc but being cautious about a firm diagnosis, even though some of the disc tissue is visible. That is, unless the diagnosis is mentioned later in the text. If it's mentioned later, then this piece is building up a case to support the diagnosis.
