These are simply "follow up" visits - usual element of any clinical trial or standard protocol for many treatments - i.e. see all patients again after a set time interval after finishing the treatment (so that data could be comparable)

protocol-based tracking & monitoring

It's not monitoring protocol, but the other way round: protocol-based monitoring of patients.

disagree Daryo : "protocol-based monitoring of patients" - yes but adding "tracking" spoils it completely - I very much doubt that these patients will be accessorised with GPS tags or that knowing their whereabouts at all times is of any importance.

I don't preface the tracking with remote as in the first web ref. and the asker from Pompey has chosen her own answer anyway.
