This is from the cover page of a court witness statement.

The text reads:

"In esito a quanto richiesto con delega in riferimento, in allegato alla presente si trasmette il verbale di sommarie informazioni rese da XXX in atti indicato"

I know it refers to the result/response of a request, but I do not know what this phrase means.

I'll be very careful with the POA version. Delega is a MUCH wider semantic field. For example, our town (Bomarzo) municipal policeman has retired and now a policeman in near-by Soriano has "la delega per Bomarzo" (the bastard comes twice a week to distribute fines!). But it's certainly not POA. There is a minister of Rapporti Con Parliamento con la delega per Rapporti con la Gioventù e le Politiche Sociali, but POA it certainly ain't. Etc.
