Sule Hanim's translation above is basically correct (except it should be "leave" not let) but in my opinion not appropriate for a nursing home slogan. If you say: leave your worries behind, it sounds to me as though you want to get rid of whoever you put there.
That's why I would chose a more loose translation. I will try to think of more options and add them later on.
